Manager – Commissioning

Brisbane South PHN is seeking a dynamic and experienced leader to drive the commissioning portfolio across the organisation to facilitate health systems improvements in Brisbane south. Program Officer – Strengthening Health Assessments for Children in Care

The Strengthening health assessment pathways for children and young people in out-of-home care project is a joint collaboration between Queensland PHNs and the Department of Child Safety, Youth and Women aimed at improving GP and primary health assessment and response to children in care. This is a full time fixed term role (9 months). You will work with the Program Coordinator – Child, Youth and Family to improve integration, including information sharing and case coordination, between Child Safety, families, carer and primary health care teams for children and young people in care. Specialist Role – Aged Care System Navigator Project

Brisbane South PHN is seeking a passionate and experienced professional to provide specialist support for vulnerable people to better engage with the Aged Care system. This is a full time role until June 2020. You will work closely with our service delivery partners, health and social services and provide “hands on” support to address barriers and enable access to aged care services. Project Coordinator – Aged Care System Navigator Project

Brisbane South PHN is seeking a dynamic and highly organised project coordinator, who will work with service delivery partners to establish and deliver aged care system information and community hubs that provide aged care system navigation support. This is a part-time 0.5 FTE role until June 2020. In this position you will coordinate the local initiatives trialling different system navigator models, working closely with community organisations and stakeholders across the health, community and social services. Program Coordinator – Palliative Care

Brisbane South PHN is seeking a dynamic and experienced project coordinator to lead the Greater Choice for at Home Palliative Care project.
