Homeless to Home Healthcare After Hours Service
Homeless to Home Healthcare After Hours Service is part of an integrated multi-disciplinary outreach team operating 7 days per week 5pm – 11pm. Operated by Micah Projects, it delivers health and housing support to people who are rough sleeping, to those who have made a transition from homelessness to housing, and who require ongoing and consistent social support and healthcare.
Street to Home Community Health Nurse
Micah Projects’ Street to Home Community Health Nurse operates 9am – 5pm, Monday to Friday providing complex care, chronic disease management and direct nursing services to homeless and vulnerably housed people.
After hours options
Brisbane South PHN has a dedicated website called Emergency Choices to guide consumer on when they should go to the Emergency Department (ED) and when they shouldn’t. This includes many free alternatives including the Healthdirect Symptom Checker, 13 HEALTH telephone service, after hours services and a list of local GPs for consumers to access.
