Residential aged care access

Residential Aged Care Access (RACA) provides access to specific allied health services for residents of residential aged care facilities, and assists to support RACFs in developing a collaborative approach with the allied health professionals for quality resident care. Allied health professionals have been engaged via expressions of interest to provide specified services. Pre-arranged packages of care are delivered at a given RACF, residents are referred, or referral on an individual basis depending on the needs of the facility or the individual resident. Brisbane South PHN recognises that access to health services is important for the quality of life of older people. Older adults are high users of acute services, so ensuring primary care services are accessible, available and affordable helps improve quality of life. Blue Care has commenced directly managing the Residential Aged Care Access (RACA) program on behalf of Brisbane South PHN and accepts referrals to this program.
