Brisbane South PHN is holding a free Building Digital Health: CAT4 The Coding Cycle of Chronic Disease session at The Glen Hotel.

Identifying patients with a possible chronic disease can ensure they are provided with the right care at the right time. The CAT Plus software suite can help you identify patients who may have fallen though the gaps and are missing from your current chronic disease registers. This event will provide you with the opportunity to see the CAT Plus software suite in action. You will also have the opportunity to discuss with your peers strategies to improve your practice’s data quality.

Guest speaker PenCS Training Programs Manager Matthias Merzenich will discuss how to:

  • easily identify patients who could be at risk of developing chronic disease
  • build chronic disease registers
  • identify revenue potential in the area of chronic disease management
  • implement change in your practice.
