Brisbane South PHN has established a free and new Community of Practice (CoP) to support health practitioners who work with older adults or have a keen interest in learning more about dementia.

All primary health care professionals and other interested services are invited to join the Older Adults CoP to learn with other like-minded practitioners, further develop local referral networks, and participate in information sharing to improve the health of older adults in the Bayside community.

Dementia is the term used to describe the symptoms of a large group of illnesses which cause a progressive decline in a person’s functioning. It is a broad term used to describe a loss of memory, intellect, rationality, social skills and physical functioning. Dementia is one of the fastest growing conditions in Australia, with incidence expected to rise steadily in coming years. It is the second leading cause of death in Australia. Dementia Australia regional director Sandra Nugent will deliver the presentation.

General practitioners, practice nurses, allied health professionals and community pharmacists play a crucial role in assisting their patients with dementia to find specialised care, support, counseling and information.
