Palliative care

The Greater Choice for At Home Palliative Care measure, facilitated through the Brisbane South PHN At Home Palliative Care project, aims to improve the provision of palliative care and end-of-life services across the general population with an initial focus on the specific needs of local multicultural and refugee communities within the Brisbane South PHN region.

It is anticipated that an evidence-based General Practitioner shared care model will be the focus of co-design activities. Using an evidence-based approach will ensure GPs remain central to the provision of palliative care services whilst also being supported by an interdisciplinary team, and will support the integration of care between different providers within and across sectors.
