Older adults are high users of acute services, so ensuring primary care services are accessible, available and affordable helps improve quality of life.

What Brisbane South PHN is doing
We are pleased to introduce to you the Older People’s Health and Wellness Strategy 2019 – 2024. The Strategy is a joint initiative between Brisbane South PHN and Metro South Health to address the health needs of older people in the Brisbane south region using a coordinated approach. The Strategy has been informed by extensive stakeholder engagement and local research. The vision for this strategy is ‘older people in the Brisbane south region experience a greater quality of life through safe connected and coordinated person-centred health care in an age friendly community’. In order to successfully achieve the vision, four key priorities have been identified:
- Facilitate connected person-centred care
- Enable evidence based, safe and quality care
- Improve health outcomes for vulnerable communities
- Build an age friendly community
The Strategy will help guide organisational and service development in the Brisbane South region and provides a framework for collectively responding to policy and health sector reform. A Strategy Committee comprised of members from Metro South Health, Brisbane South PHN, and key organisation representatives will guide and monitor the Strategy implementation over the coming five years.

The Carer Gateway is a national online and phone service that provides practical information and resources to carers and health professionals who work with patients and carers.
The interactive service finder helps carers connect to local support services, and the site includes a section for Aboriginal and Torres strait Islander people in addition to the culturally and linguistically diverse people.
