Brisbane South PHN commissions mental health, suicide prevention and alcohol and other drug services designed to provide flexible support that is best suited to an individual’s needs. Our work aims to improve the health and wellbeing of our community through investing and supporting integrated health care systems.
In 2018 Brisbane South PHN embarked on a co-design journey to inform our vision for an integrated person centred service model enabled by a strong primary health sector – building on the strengths of our existing programs, while addressing service barriers to improve accessibility. This valuable process resulted in the design and commissioning of a new person centred service model based on a philosophy that health is a state of physical, social, cultural, historical and emotional wellbeing and as such provides integrated responses to physical, cultural, spiritual and mental health needs. All Brisbane South PHN commissioned mental health services share a common set of core principles.
The new services model is place-based, with three sub-regions – Brisbane (Princess Alexandra Hospital catchment area), Logan/Beaudesert (Logan Hospital catchment area) and Redlands (Redlands Hospital catchment area).
