A person may be experiencing homelessness for many reasons, but ensuring they are provided quality information and connected to services to provide advice and support them during this time is a priority for Brisbane South PHN.
Homeless Hotline
Patients or clients experiencing homelessness or at risk of homelessness can contact the Homeless Hotline to obtain confidential advice and receive information about an immediate safe place to stay, practical assistance, such as where to get meals, showers and clothing, and support.

Street to Home Outreach Services
Street to Home Outreach Services is delivered by Micah Projects. Micah Projects is a not-for-profit organisation providing health, housing and social support to people experiencing homelessness and to those who have made a transition from homelessness to housing, but who require ongoing and consistent social support and health care.
After Hours Services
The Street to Home After Hours Nurses and Support and Advocacy workers provide integrated outreach services, operating across Brisbane, seven nights per week from 6 pm – 12 pm.
Community Health Services
The Street to Home Community Health Nurse provides direct nursing care, care coordination and case management of vulnerable people with complex health and social support needs.
Logan Street Doctor
The Logan Street Doctor provides outreach primary health care services to people experiencing homelessness within the Logan community. Services operate seven nights per week from 6 pm Monday – Friday and from 4 pm Saturday and Sunday.
Ask Izzy
Ask Izzy is a free, anonymous online portal that helps people find the services they need immediately and nearby, such as housing, meals, shower and toilet facilities. The service is easy to use and accessible from phones on the Telstra mobile network even if they don’t have credit.
