SpotOnHealth HealthPathways
Metro South Health and Brisbane South PHN are committed to supporting general practice. Resources from both organisations enable the delivery of SpotOnHealth HealthPathways in the Brisbane South region. Partnerships with Mater Health Services and Children’s Health Queensland ensure that care pathways can be localised for a wide range of patient care needs relevant to our region.
SpotOnHealth HealthPathways is an online clinical decision support tool that reflects evidence-based health care and best-practice approaches. The tool aligns with local health priorities, while still allowing for clinical autonomy and supporting patient-centred care. SpotOnHealth HealthPathways provides clinicians with information outlining the assessment, management and referral options for a wide range of health conditions and is designed to help them navigate the complex primary, community, and acute care systems of the greater Brisbane south region.
SpotOnHealth HealthPathways is being progressively localised through collaborations between General Practitioners, hospital clinicians and a broad range of other health care professionals and provides a central location to support clinical practice and minimise the variation in treatment approaches within our health system. The tool is designed to be used at point of care primarily by general practitioners but is also available to other health professionals. The website is not available for use by patients, however there are local resources for clinicians to provide to patients.
SpotOnHealth HealthPathways is designed to save clinicians time during a consultation and boasts a range of benefits including:
- best available information on how to assess and manage common clinical conditions, including when and where to refer patients
- easy online access to clinical and patient resources for in-consult use
- peer-reviewed information
- localised to our region
- integrated and concise.
HealthPathways is based on an initiative from the Canterbury District Health Board (CDHB) in Christchurch, New Zealand, where it has contributed to significant improvements in the way hospitals and general practice share the care and management of patients.
