Making Every Health Assessment Count

Health assessments are much more than a Medicare item. They provide an opportunity to identify health risks, support early intervention, strengthen preventive care, and improve patient outcomes.

Join us for our upcoming Nurse Network meeting as we explore the real value of health assessments in general practice. We’ll discuss how health assessments can uncover unmet health needs, support chronic disease prevention and management, and create opportunities for patient care.

The Quality Improvement Team will share practical strategies to help you identify eligible patients, improve uptake, and streamline workflows to support the delivery of health assessments.

We’ll explore practical ways to make health assessments more meaningful and effective, including how to get the most from your practice’s health assessment templates and identify follow-up recommendations. You are welcome to share with us what works well in your practice or the issues you facing.

Learn about the resources available through Brisbane South HealthPathways and we’ll show you where to find tools and information to support assessment, referral, care planning, and patient education.

Whether you’re new to health assessments or looking to strengthen your current approach, this session will provide practical tips, useful resources, and ideas you can take back and implement in your practice straight away.
