HealthPathways is a digital tool designed to help clinicians navigate the complex health care systems of the Brisbane South, Redlands, Logan, and Scenic Rim catchments. It is an initiative of Metro South Health and Brisbane South PHN, in partnership with Mater Health Services and Children's Health Queensland.

Brisbane South PHN, Metro South Health, and Children’s Health Queensland are working collaboratively with local community service providers and representatives to develop a unified approach to improving health outcomes for the Pasifika and Māori community in our region. This cross-sector collaboration is guided by the Pasifika and Māori Health and Wellbeing: A Strategic Framework and Action Plan for Brisbane South 2020-2025. Community workshops are held to ensure community members are involved across the implementation of the Strategy.

Brisbane South PHN is working closely with the Southern Moreton Bay Islands (SMBI) community to improve the health and wellbeing of children aged from birth to 8 years old, including improving maternity care, through the SMBI Community Impact Initiative. This initiative delivers a community-driven, cross-sector, place-based approach to improve the health and development of children residing on the islands.

There are numerous challenges associated with delivering health care services to the SMBI area, and health data indicates children residing on SMBI experience a high than average percentage of development vulnerabilities. For this reason, Children’s Health Queensland, Metro South Health, Brisbane South PHN and the SMBI community are working together to deliver the SMBI Initiative.

Brisbane South PHN in partnership with Metro South Addiction and Mental Health Services (MSAMHS) have developed the Working Together Differently – Brisbane South Mental Health, Suicide Prevention and Alcohol and Other Drug Foundation Plan 2020-2022. The Foundation Plan is the framework for how Brisbane South PHN and MSAMHS will work together in the short term to lay the foundations for working together differently in the future. The Foundation Plan describes the key actions and agreed priorities for new and joint services and service planning for the future and provides the link between the work undertaken by the partners on the priority strategic domains of the Brisbane South Mental Health, Suicide Prevention and Alcohol and Other Drug Strategy 2019-2022. The Foundation Plan will inform future collaboration with a commitment to developing a comprehensive 5-year Joint Regional Plan by June 2022.
