Social workers in general practice

Brisbane South PHN recognise the impact of social issues on health.  We are working with practices to help improve social health by piloting models to incorporate social workers into general practice teams.  

Enabling patient access to on-site social workers at their general practice can enable a holistic assessment of health concerns and help address social determinants of health such as lifestyle, housing, and family stressors.
