The Homeless Access Program

It aims to improve access to quality primary care services for people experiencing homelessness (or those vulnerably housed) by providing access to coordinated and integrated primary care services.

Health related objectives of the program include reduction in burden of poor health for the target population, improve health service navigation within their LGA, and reduction in uptake of emergency services. This is achieved by the following:

  • respond to referrals requesting assistance to address presenting health concerns identified among homeless and vulnerable housed persons within the Brisbane and Logan City Council boundaries

  • undertake triage and assessment services

  • nursing care based on referrals and assessments by phone, in public spaces, motels, boarding houses and in the home

  • public space – engaging with and working with people sleeping rough to:

    • manage public intoxication to reduce incarceration.

    • move people into a place of safety, and

    • support people to access permanent housing.

Micah Projects (Brisbane)

Micah Projects have been commissioned to work alongside Brisbane South and Brisbane North PHNs to support people experiencing homelessness in Brisbane.
