Activating pharmacists to reduce the frequency of medication-related problems (ACTMed): a stepped wedge cluster randomised trial

The ACTMed project was developed by researchers at the University of Queensland, the University of Melbourne, Monash University, and Griffith University, in partnership with Brisbane South PHN, the Pharmaceutical Society of Australia, the Australian Digital Health Agency, the National Aboriginal Community Controlled Health Organisation, MedAdvisor, and the Pharmacy Guild of Australia.
