The Social Worker will work with adults from culturally and linguistically diverse (CALD) backgrounds and/or people living in rural locations with complex needs who experience poorer health outcomes compared to the general population because of unmet social support needs. The social worker will provide a direct service through interpersonal practice, support and advocacy via physical presence (through on-site provision of service in one or more general practices in the Logan/Browns Plains area) or virtual (by providing a direct link to general practice via a remote service).
Requirements:
Bachelor’s degree in social work
Current Member of the Australian Association of Social Workers and Trademark accreditation
Minimum of 3 years of experience in a social work role
Willingness to work with clients who experience complex health challenges, their family and community members and service providers to identify solutions
Have an understanding of person-centred practice
Strong networks and knowledge of local support services which people may be referred to
Willingness to network and develop effective working relationships including with external service providers and internal stakeholders
Strong knowledge of the challenges faced by CALD persons, government policy and legislation, and professional standards and codes of ethics as they pertain to working with people
Demonstrated ability to work effectively with other professionals in a multi-disciplinary team, as well as consult, liaise and work with a wide range of external stakeholders involved in the provision of health, mental health, and chronic health conditions.
Ability to work independently, with strong organisational and time management skills.
Highly developed interpersonal communication and decision-making skills, coupled with the ability to involve other staff actively and appropriately, clients, their families, and significant others in the delivery of service where appropriate.
Excellent communication, interpersonal, and organisational skills.
Ability to work effectively both independently and as part of a multidisciplinary team.
National Workers Screening check or current Yellow Card.
