Activity Work Plans

Brisbane South PHN develops Activity Work Plans outlining the work we are doing to address the region’s needs and service gaps identified through our Health Needs Assessment

  • ensuring commissioned services are providing effective, evidenced-based care

  • improving people’s access to quality health care

  • supporting the coordination of health services

  • responding to the unique needs of our region to ensure long term improvements.
