Activity Work Plans
Introduction
Brisbane South PHN develops Activity Work Plans outlining the work we are doing to address the region’s needs and service gaps identified through our Health Needs Assessment.Â
Activity Work Plans (AWPs) are submitted to the Department of Health and Aged Care as part of our funding requirements. Our AWPs provide the Department with further information on how we are:
ensuring commissioned services are providing effective, evidenced-based care
improving people’s access to quality health careÂ
supporting the coordination of health servicesÂ
responding to the unique needs of our region to ensure long term improvements.
