Introduction

Brisbane South PHN develops Activity Work Plans outlining the work we are doing to address the region’s needs and service gaps identified through our Health Needs Assessment. 

Activity Work Plans (AWPs) are submitted to the Department of Health and Aged Care as part of our funding requirements. Our AWPs provide the Department with further information on how we are:

  • ensuring commissioned services are providing effective, evidenced-based care

  • improving people’s access to quality health care 

  • supporting the coordination of health services 

  • responding to the unique needs of our region to ensure long term improvements.

Our health priorities

The following priority areas are identified and addressed throughout our Activity Work Plans. 

Commonwealth-informed priorities: 

  • Mental health

  • Aboriginal and Torres Strait Islander health

  • Population health

  • Health workforce

  • Digital health

  • Aged care

  • Alcohol and other drugs

Local priorities:

  • Multicultural health

  • First 2000 Days 

  • Southern Moreton Bay Islands (SMBI) programs

  • Suicide prevention

  • Chronic diseases

  • GP Workforce Prioritisation and Planning
