Call for GPs to join the Palliative Care Shared Care Project
Brisbane South PHN and Metro South Palliative Care Service (MSPCS) are inviting local general practitioners to express their interest in participating in the Palliative Care Shared Care Project. This project will inform, build, test and evaluate an improved shared care model that supports people receiving palliative care at home or in the community, including in Residential Aged Care Facilities (RACF).
The Palliative Care Shared Care Project aims to enhance the quality and availability of community-based palliative care while enhancing GPs’ capabilities in delivering this support to their patients.
GPs will work together with a Nurse Practitioner to coordinate patient access (based on service eligibility criteria) to MSPCS Multidisciplinary Team specialist, equipment, and domiciliary nursing services. Each participating GP will be involved in delivering shared care to at least 2 palliative patients over the duration of the project.
GPs will be remunerated for their involvement in the project.
