Improved cardiovascular risk calculator for your practice

Did you know that the cardiovascular disease risk (CVD) calculator in your clinical software has had a makeover?

To check out the changes to the CVD risk calculator and boost heart health in your patients, see the Boosting Heart Health: Increasing use of the CVD risk Calculator self-paced activity.

Brisbane South PHN has a new self-paced quality improvement activity which steps you through using Primary Sense (PS) to identify 10 patients who have a high or moderate risk of CVD. From there it will show you how to use the CVD risk calculator to record your patient’s CVD risk.

 Important update for Best Practice users

A reminder to ensure that your practice is running the latest version of Best Practice – Premier Spectra SP1 Revision 3.  This update ensures that your practice remains compliant and prepared for:

  • the new Australian Immunisation Register (AIR)

  • November Medicare updates, including the Bulk Billing Practice Incentive Program (BBPIP)

  • long-acting reversible contraceptive (LARC) item updates

  • improved workflows for managing mental health care plans.

PHASES launches statewide — Be part of a new primary care initiative to find cardiovascular risk earlier and improve patient outcomes

Queensland-based general practices and Aboriginal Community Controlled Health Organisations are invited to the statewide launch of Preventing Heart Attacks and Stroke Events through Surveillance (PHASES).

 Join Professor Kim Greaves, PHASES Clinical Lead, Cardiologist and Epidemiologist, for a practical 60-minute briefing.

Learning outcomes
By the end of the session, participants will:

  • learn how Primary Sense™ supports early identification and optimisation of management through clinical auditing

  • be able to identify practical steps to collaborate with the PHN on project implementation and evaluation.
