Digital Tools for smarter CVD prevention in general practice
Want to improve patient outcomes and run a more sustainable practice? Discover how digital tools can support accurate billing, streamline care planning, and strengthen long-term sustainability – all the while enhancing your practice’s approach to cardiovascular disease (CVD) prevention.
Join us for a practical webinar focused on using digital tools to drive smarter, more proactive care in general practice, as part of the ongoing PHASES (Preventing Heart Attack and Stroke Events through Surveillance) series.

Update: All patients can receive a GP Chronic Condition Management Plan (GPCCMP) and eligible reviews from their usual general practitioner (GP)
Key details:
Patients can access chronic condition management (CCM) services through their MyMedicare practice, or if not registered, their usual GP.
MyMedicare registration is optional, and patients are encouraged to register, particularly if they believe it will support continuity of care with their usual practice.
Patients with a chronic condition may benefit from registering as MyMedicare is designed to help strengthen continuity of care with their regular practice.
What practices need to know:
To avoid your practice having a declined MBS claim, you can check a patient’s registration status in the ‘Your Practice’ section of PRODA, prior to the appointment.
While there is no financial or operational disadvantage for practices when a patient is not registered with MyMedicare, registration can support stronger continuity of care by formalising their relationship with your practice.
MyMedicare formalises the patient–practice relationship, so it is important that patients understand they need to see your practice for chronic condition management items.
CCM arrangements remain unchanged for patients who are not registered in MyMedicare. Consistent with long-standing requirements, assessments and reviews must be provided by the patient’s usual GP.

Free GP education module: Assessing Fitness to Drive (AFT)
Access free, practical guidance to support fitness to drive assessments and patient conversations about driving.
This free, CPD-accredited AFT learning module for GPs has been developed to enable proactive management of fitness to drive and patient-centred conversations. This module is free for GPs until 31 May 2026.
The module provides practical guidance on applying the national Assessing Fitness to Drive (2022) Standards in everyday clinical practice. On completion of the module, participants will have covered the:
foundational knowledge on how medical conditions, injuries and disabilities may affect a patient’s capacity to drive
principles that underpin the assessment and management of fitness to drive.
communication approaches to best discuss options for maintaining safe driving, including conditional licences, driving adjustments, and, when necessary, planning for a supported transition away from driving.
