Social workers in general practice: join the program in Redland City
We are currently recruiting for a full-time social worker to be based in Redland City. This role is best suited to an experienced professional with a strong background in complex systems and multidisciplinary teams. It is not suitable for new graduates or those with limited experience.
The project requires a generalist social worker. While accreditation as a mental health social worker is welcome, a singular focus on mental health counselling falls outside the scope of this role.
This position is part of a pilot project embedding social workers into general practice.

Brisbane South PHN has been piloting social workers in general practices across the region. Feedback from general practitioners (GPs) participating in the pilot has been extremely positive, with the majority of GPs reporting there were noticeable benefits to having a social worker embedded in their practice.
Social workers in general practices have proven to be instrumental in helping patients by addressing social determinants of health, such as lifestyle, housing, and family stressors.
Since the pilot’s launch, 94 patients from general practices have had an in-practice social worker help them navigate complex life challenges that are influential in affecting patient health and wellbeing, such as:
housing applications
National Disability Insurance Scheme (NDIS) access
mental health services, and more.
