ADHD Collaborative Care Pathway: Tell us your feedback!
Brisbane South PHN partnered with Metro South Health, Children’s Health Queensland and Metro North Health to develop and implement the ADHD Collaborative Care pathway. The pathway is a collaborative care model that involves the transfer of care of selected children or young people with ADHD from public hospital paediatricians to their GPs for ongoing review for medication management.
Resources were developed to support this pathway including revised ADHD HealthPathways pages, tailored discharge letters and the ability to request advice from paediatricians for children and young people on the pathway.
The online survey takes approximately two to 10 minutes to complete. Your responses will not be identifiable. Responses will assist us to improve the pathway and resources.
The survey closes Monday 28 September 2026.

Feedback from GPs will help inform ongoing improvements to the pathway and supporting resources.

Your insights will help partners better understand how the pathway is working in practice and identify opportunities to further support shared care for children and young people with ADHD.
