The upcoming changes to Medicare Benefits Schedule (MBS) Chronic Disease Management Arrangements are confirmed to commence from Tuesday 1 July 2025. 

The changes aim to support continuity of care, reduce administrative burden, and enable a more flexible, person-centred approach to chronic condition management.  

You can access new fact sheets covering key aspects of these changes, such as: 

  • the new planning and referral processes 

  • how allied health access will work 

  • the transition arrangements in place for existing patients.
