Enhancing end-of-life care through collaboration: ELDAC Linkages Program now open for expression of interest
The End of Life Directions for Aged Care (ELDAC) Linkages Program is a national initiative helping aged care service providers strengthen their approach to palliative care and advance care planning.
Funded by the Australian Government Department of Health, Disability and Ageing (formerly the Department of Health and Aged Care), the program supports aged care services to improve internal practices and build meaningful partnerships with primary and specialist palliative care providers.
The program, delivered by Queensland University of Technology (QUT), aims to improve circumstances for older people at the end of life through better coordination and timely access to care.
How the ELDAC Linkages Program can support aged care services
The ELDAC Linkages Program is currently seeking expressions of interest (EOI) from a range of residential aged care and home care package providers.
Participating services receive tailored support from an experienced ELDAC facilitator, along with access to practical tools and resources to:
review and strengthen palliative care and advance care planning processes
build staff capability to support people nearing the end of life
develop and implement quality improvement initiatives
strengthen local partnerships across the care continuum.
Eligible services may also receive an administrative grant of up to $20,000 to support their participation. Expressions of interest are open on a rolling basis.
