Expression of Interest: Improve patient-practitioner communication

Brisbane South PHN invites general practices to submit an Expression of Interest (EOI) to participate in our Effective Efficient Engagement (EEE) project. The project is expected to commence in February 2024 and conclude July 2024.

About the Effective Efficient Engagement (EEE) project

The EEE project is seeking participants to help enhance and improve communication between patients and healthcare practitioners during appointments.

The project will trial 2 resources which have been designed to help patients better prepare for their healthcare appointments and will aid in improving streamlined communication between patients and healthcare practitioners. The resources provided to practices for this trial are:

  1. The My Health Journal
  • My Health Journals are designed to help patients prepare for appointments and keep track of their follow-up instructions. Developed through intensive community consultation, and successfully trialed in healthy ageing hubs, the journals have received positive feedback through the Brisbane South PHN Health Literacy Pilot project.   1. HealthShare Digital’s BetterConsult software
  • This software is used to send simple questionnaires for patients to fill out. The questionnaires highlight patient attitudes and understanding of their holistic wellbeing as well as an overview of their presenting condition(s). 98% of patients reported having no difficulty completing the BetterConsult questionnaires.

How will the resources be provided to practices?

Physical copies of the My Health Journal will be given to practices for GPs or practice nurses to give directly to their patients. Journals will also be located in practice waiting rooms for patients to freely take. Brisbane South PHN will provide an ongoing supply of My Health Journals.

The HealthShare Digital team will schedule a training call with the practice and onboard the practice team with the BetterConsult software. The session is 60 minutes and will be scheduled to align with the practice’s availability. Brisbane South PHN Evidence Translation and Innovation team will provide each practice with ongoing support throughout the 6-month project.
