Free multidisciplinary mental health support in your practice. Express your interest today

Brisbane South PHN is partnering with general practices in Beenleigh, Eagleby, Loganlea, Inala, Forest Lake and Oxley to help provide a free, in-practice multidisciplinary primary mental health service to support patients who are experiencing severe or complex mental health challenges.

Through this initiative, professional community-based care providers will work alongside your practice to support your patients who need more intensive, coordinated care.

If your patients might benefit from free access to social workers, mental health nurses, allied health professionals or peer support workers, and your general practice is in the aforementioned locations, we encourage you to apply.

This PHN-led initiative is designed to support adult patients experiencing severe and complex mental health challenges, including patients:

  • with ongoing high-level clinical needs

  • who frequently use hospital services

  • co-occurring health conditions, including other chronic conditions

  • with functional impairment (including social challenges)

  • facing barriers to accessing or using mental health supports (including insecure housing or financial challenges).

How to participate in this free, multidisciplinary mental health support initiative

This opportunity is available to general practices in:

  • Beenleigh

  • Eagleby

  • Forest Lake

  • Inala

  • Loganlea

  • Oxley.

As part of the initiative, participating general practices will need to:

  • provide a room for onsite appointments, either together with GPs in consultation rooms or separately (approximately 1 day per week, or as required)

  • participate in shared care activities

  • contribute to national/local evaluation (non-identified data only).
