New models of care

Shared medical appointments — Improve access to timely, tailored healthcare for your patients

Just one hour of GP time can multiply impact by delivering service to around 10 patients. Shared medical appointments are supported by practice nurses or allied health professionals to streamline:

  • recording patients’ history

  • prioritisation of reason for visit

  • note taking for each patient.

Quality improvement

Improve pneumococcal and COVID vaccinations for vulnerable patients

Eligible patients: 70+ years for pneumococcal and 75+ years for COVID 19.

In partnership with GoShare, Brisbane South PHN invites your practice to register for the pneumococcal and/or COVID-19 Vaccination Recall Programs. Support is provided to identify eligible patients.

The Vaccination Recall Program has previously delivered great results for practices and their patients.

Participation in the recall program activities:

  • is available to practices with MedicalDirector or Best Practice software

  • includes an unlimited number of free SMS credits to recall patients eligible for the vaccination/s

  • only takes 15 minutes of a practice staff member’s time to complete.
