Closing soon: Help shape the future of diabetes care
Metro South Health invites general practitioners (GPs) and practice nurses to share their insights through a short survey. Survey feedback will be used to shape how diabetes care is delivered across our region.
The survey takes less than 10 minutes to complete. All responses are confidential.
Your feedback will help to:
improve support for general practice from hospital specialists
strengthen how general practices and Metro South Health can collaborate together
address local challenges in diabetes care.
As a thank you, participants will be eligible for a 50% discount to the LEADS Diabetes Symposium taking place in November 2026.

MyMedicare aims to support continuity of care
MyMedicare is a voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner and primary care teams.
Information for practices
Participating in MyMedicare does not limit where a patient can go for immediate or unplanned care.
For ongoing and planned care, such as chronic condition management plans and reviews, patients should attend the practice they are registered with under MyMedicare, to support continuity and coordinated care.
Practices can continue to see any patient for care, regardless of whether they are registered in MyMedicare.
What your patients should know about MyMedicare
Reinforce that while patients can seek urgent or same-day care anywhere, MyMedicare strengthens continuity of care by supporting patients to return to their regular practice for ongoing and planned care.
Explain to patients that they need to attend their MyMedicare registered practice for chronic condition management, care plans and reviews.
