Help shape the future of diabetes care — Have your say
Metro South Health is currently reviewing how diabetes care is delivered across our region to ensure you and your patients receive effective care and support.
To help inform this work, Metro South Health invites general practitioners (GPs) and practice nurses to complete a short survey to help:
assist Metro South Health to understand your learning needs, confidence, interests and preferences for education that meets GP and practice nurse needs
understand your current needs in the management of diabetes patients
how future diabetes training and specialist support is best designed for primary healthcare clinicians and patient health and wellbeing outcomes.
Your feedback will help to:
inform the development of education and resources to support general practices
enhance collaboration between primary care, hospital, and community services
identify key challenges and opportunities in diabetes management
shape how hospital specialist services can help support you with the care of your diabetes patients
The survey takes less than 10 minutes to complete and responses will be kept confidential.
As a thank you, participants will be eligible for a 50% discount to the LEADS Diabetes Symposium to be held on 13 November 2026.

Our self-paced Quality improvement (QI) activity can help your practice increase the rate of blood pressure recordings for patients with diabetes. By ensuring patients with diabetes have a recent blood pressure result recorded, healthcare teams can better understand absolute cardiovascular risk and provide appropriate preventative and management care to patients who need it most.
