Mater's Multicultural Health Coordination Program secures funding to continue

Mater has announced it can continue delivering the Multicultural Health Coordination Program through ongoing funding from Queensland Health. The program is free for people of all ages, from multicultural backgrounds, who live in Brisbane, Logan, and Ipswich and have complex health and/or social needs.

How can the Multicultural Health Coordination Program benefit your patients?

Mater's Multicultural Health Coordination Program (MHCP) is a care coordination service that aims to increase the health and health literacy of people from multicultural backgrounds with complex health needs. The MHCP helps to strengthen the patient’s connection with their primary care provider, tertiary care provider, and/or community services while increasing the patient's understanding of the health system.

The MHCP team includes clinical nurses, a social worker and program staff who work in partnership with the patient, the patient’s family and support system, their GP and other health and community services. The program works with patients for both extended care and brief interventions as needed.

Free, wrap-around health support for eligible patients

The MHCP's core services include:

  • assessment and care coordination
  • family and carer support
  • provision of health literacy education (health conditions, medications, and health system navigation)
  • facilitating patient access to community support organisations, GPs and medical specialists.

Services are being delivered in general practices, hospital settings and in the community, including through home visits, depending on need.

Referral criteria and pathway

Referrals can be made by:

  • General Practitioners
  • hospital EDs, inpatient or outpatient services
  • community health services
  • social care providers
  • community members, including self-referral.

Referrals can be made by completing this referral form. Referral letters from all clinicians are also welcomed.
