MyHeart MyLife patient support program is now live

The Heart Foundation’s new MyHeart MyLife 12-week patient support program is now live and available for all adults living with coronary heart disease in Australia. The free, evidence-based digital program aims to improve the physical and emotional health and wellbeing of people living with coronary heart disease.

Participants will receive supportive patient information, practical tips and expert guidance delivered via text messages, emails, engaging video content and bite-sized articles to complement the clinical care that you provide to your patients.

Launched in August 2024 at Parliament House in Canberra by the Assistant Minister for Health and Aged Care, the Hon Ged Kearney MP, the program is designed to support the nearly 600,000 Australians living with coronary heart disease, helping them to manage a complex, chronic condition while also navigating the daily challenges of life.

About the MyHeart MyLife patient support program

The Heart Foundation collected extensive end user feedback and listened to the needs of people living with heart disease, their carers and healthcare professionals working across hospital and primary care settings to design the program.

The free program offers tailored journeys for recently diagnosed patients, those who have been living with heart disease for some time, carers, and those residing in rural and remote areas of Australia.

Patient information and support to assist with physical and mental recovery is delivered via multiple formats at a time and pace that suits each participant.

Key features of the 12-week program include:

  • an interactive online dashboard where people can access short articles and informative videos when they are ready
  • supportive text messages and emails aimed to motivate and inspire
  • access to the MyHeart MyLife online peer support community
  • complementary patient education resources available for order or download.
