The University of Queensland is seeking GPs and community pharmacists to join a workshop for a study investigating the best way to handover information about medicines when people leave the hospital.

When a patient is discharged from hospital, it is essential that their GP and community pharmacist are informed
of changes to their medicines to continue their medicine management in the community. However, the
handover of medicine information can often be inconsistent.

Purpose of the optimising medicine information handover post discharge (OPTMED-D) study

The disconnect between different health services in hospital and primary care settings impacts patient safety, leading to hospital readmissions and increased cost to the healthcare system. The ideal outcome is for patients to experience seamless medicine information handover during transitions of care from acute care settings to community care.

This study is seeking GPs and community pharmacists to attend a 1-hour workshop to discuss solutions to
improve transitions of care between hospital and primary care settings. This opportunity would suit a GP or
pharmacist that has experience with discharge summaries and medicine information handover from hospital.

Time and location

The University of Queensland are hosting an online workshop Wednesday 31 July 2024, from 7pm to 8pm for General Practitioners and community pharmacists within the Brisbane South PHN area.

Remuneration

Remuneration will be provided to all participants, according to the Brisbane South PHN renumeration policy.
