Put your heart Into Heart Week 2026 — Be part of our Heart Health Hub Challenge
In the lead up to Heart Week (4 to10 May 2026), Brisbane South PHN invites local general practices to join the Heart Health Hub Challenge and help spark important conversations about heart health with patients.
Read on to see simple steps your practice can take to encourage conversations that can lead to a reduction of heart attack and stroke in our communities.
Be part of the Heart Health Hub Challenge — Transform your waiting room into a Heart Health Hub
As part of the PHASES Project (Preventing Heart Attack and Stroke Events through Surveillance), Brisbane South PHN has been working with general practices to strengthen cardiovascular disease prevention and improve identification of at-risk patients.
Heart Week in May provides another opportunity to build on the work of the PHASES Project, by creating an opportunity to start conversations with patients about heart health and ways to reduce the risk of heart attack and stroke.
To help spread the word, we invite your practice to take part in our Heart Health Hub Challenge this Heart Week. Even a simple display can prompt a powerful conversation about cardiovascular risk and prevention.
Decorate your practice in any way that grabs attention and encourages patients to start thinking about their heart health. Some ideas include:
red displays
heart-themed decorations
posters and conversation starters
heart health resources for patients.
Gift cards will be awarded to practices with the heartiest displays. All participating clinics will be entered into a random prize draw.

Submit your photos to Brisbane South PHN by Friday, 1 May 2026 to be in the running.
