Supporting GPs to provide quality end-of-life care: A joint initiative by Brisbane South PHN and QVAD Support and Pharmacy Service

Brisbane South PHN has partnered with Queensland Voluntary Assisted Dying Support and Pharmacy Service (QVAD SPS) to provide GPs and other primary care clinicians with resources and training to support quality end-of-life-care for their patients. 

As part of the joint project, resource packs are being mailed to approximately 800 general practices across Queensland. 

The packs include practical tools and information to support GPs in delivering high-quality, person-centred end-of-life care.

About the resource packs

These packs include:

  • the Prompts for End-of-Life Planning (PELP) Framework, to support early identification and proactive care for patients likely to be in the last year of life

  • information on PallConsult, a 24- hour 7-days-a-week statewide palliative care advice line offering clinical guidance to Queensland health professionals

  • information on the PalliMEDS app, which has been designed for generalist healthcare providers to support the management of common end-of-life symptoms in community settings

  • Voluntary Assisted Dying (VAD) brochures and My Care, My Choices advance care planning brochures for display in general practices

  • information on the ‘Voluntary assisted dying: What GPs need to know’ module, a new online education module and earn RACGP and ACRRM CPD hours.

About the ‘Voluntary assisted dying: What GPs need to know’ education module

Developed in partnership with Brisbane South PHN and Queensland Government, this free education module guides GPs on how to:

  • initiate VAD conversations with patients

  • recognise eligibility

  • refer appropriately

  • understand your legal obligations.
