The General Practice in Aged Care Incentive is coming 1 July 2024
The General Practice in Aged Care Incentive will launch Monday 1 July 2024 and will replace the current Aged Care Access Incentive.
The purpose of the General Practice in Aged Care Incentive is to support older people living in Residential Aged Care Homes (RACHs) receive regular visits and care planning services from their responsible provider and practice.
The incentive payments are for providing regular visits and care planning to older people living in aged care. This incentive is designed to be an important enabler for continuity of care and better health equity across the Brisbane South PHN region.
Eligibility details
From 1 July 2024, eligible providers and practices, registered with MyMedicare, will be able to receive the General Practice in Aged Care Incentive payments.
Practice eligibility
General practices must be an eligible practice and registered in the:
- Organisation Register
- MyMedicare program (with banking details added)
- General Practice in Aged Care Incentive program.
Patient eligibility
Patients must:
- permanently live in a Residential Aged Care Home, not including respite care
- register with the MyMedicare program and link to an eligible practice and responsible provider
- have the General Practice in Aged Care Incentive indicator selected on their MyMedicare profile by their practice.
