The University of Queensland is seeking general practitioners and community pharmacists to join a workshop for a study investigating the best way to handover information about medicines when people leave the hospital. The study is called “Optimising medicine information handover post discharge (OPTMED-D).”

•Ensure interventions developed are relevant, feasible, and desirable

Purpose
When a patient is discharged from hospital, it is essential that their GP and community pharmacist are informed of changes to their medicines to continue their medicine management in the community. However, the handover of medicine information is inconsistent. The disconnect between different health services in hospital and primary care settings impacts patient safety, leading to hospital readmissions and increased cost to the healthcare system. Our vision is for patients to experience seamless medicine information handover during transitions of care from acute care settings to community care.

This study is seeking GPs and community pharmacists to attend a one-hour workshop to discuss solutions to improve transitions of care between hospital and primary care settings. This opportunity would suit a doctor or pharmacist that has experience with discharge summaries and medicine information handover from hospital.

Renumeration
Renumeration will be provided to all participants, according to the Brisbane South PHN renumeration policy.
