Experienced Health Coaches deliver valuable support to My Health For Life (MH4L) program participants for free
My Health for Life Health (MH4L) supports people at high risk of developing chronic conditions like type 2 diabetes, heart disease and stroke to make healthy lifestyle changes.
My Health for Life Health (MH4L)'s Health Coaches are qualified, allied-health professionals who can help your patients who are 18 years and over, make healthier choices and create healthier habits. The program is delivered in 6 coaching sessions at a relaxed pace over 18 weeks.

How to refer patients to the MH4L program
