Connecting patients to Medicare Mental Health support just got easier with a new eReferral pathway

Connecting your patients to support through Medicare Mental Health just got easier, with a new eReferral form now available in your practice software.

New eReferral form available for fast, easy Medicare Mental Health referrals 

Available for general practices, the HealthLink SmartForm eReferral is embedded in participating practice software and provides a simple, secure and trackable way to refer patients directly to the Medicare Mental Health phone service for Initial Assessment and Referral (IAR) to locally available services.  
 
With an eReferral, receive confirmation the moment a referral is submitted and track its progress all in your clinical software, removing the need for follow-up phone calls on behalf of your patients.

Prefer to speak with someone? Phone 1800 595 212 for service navigation advice and referral support 

Referring clinicians and health professionals can call the Medicare Mental Health phone service on behalf of patients for support with assessment and local service navigation and referrals. Phone service staff use the IAR Decision Support Tool to establish your patient's level of need and connect them to the right local service, with a warm handover and follow-up until they are linked in with a provider. 

Along with the new eReferral form, the Medicare Mental Health phone service is the preferred referral pathway for patient mental health care and support in the Brisbane South PHN region.   

The Medicare Mental Health phone service is delivered locally by Neami National and operates 8.30 am to 5 pm weekdays, excluding public holidays. Learn more on our Medicare Mental Health phone service page.

Medicare Mental Health offers free, walk-in or online support for your patients

Medicare Mental Health is the Australian Government's national initiative connecting people to free mental health support, online, on the phone or closer to home. No diagnosis or referrals are needed to access support.  
Medicare Mental Health centres 

There are 2 walk-in Medicare Mental Health Centres in the Brisbane South PHN region, with a further 2 centres set to open in the region in 2027. The centres are in Logan and Redlands and are open 10 am to 6 pm weekdays.   
Medicare Mental Health phone service 

Your patients (or their families and carers) can also call Medicare Mental Health on 1800 595 212 directly for free wellbeing support and connection to local services.  

Medicare Mental Health Check In app 

For patients aged 16 and over with mild to moderate concerns (IAR levels 1–2), Medicare Mental Health Check In offers free online low-intensity cognitive behavioural therapy (LiCBT) programs, both self-guided or practitioner-guided with no formal referral required.
