Dementia Outreach Service offers support for your patients with dementia

The Dementia Outreach Service provides short-term clinical interventions for people living with dementia who are medically and socially at risk due to complexities related to their disease process, who live in the community, their family/carers, and service providers.

About the Dementia Outreach Service

Comprised of a small team of specialist dementia nurses and a social worker, led by a Nurse Practitioner, the Dementia Outreach Service aims to ensure the complex needs of people with dementia are being met optimally at home so that the best quality of life can be achieved for people with dementia and as well as their support networks.

The service also supports people with dementia to avoid premature Residential Aged Care Facility entry, non-medical hospital presentations, admissions, and readmissions caused by issues related to their dementia including behavioural and psychological symptoms of dementia and caregiver burden.
