New quality improvement activity: Supporting glycaemic control for patients with diabetes and a HbA1c more than 7.0%
Diabetes guidelines recommend a target HbA1c of less than 7.0% (53 mmol/mol) for patients with diabetes. There is strong evidence that better glycaemic control (7.0% and under) can reduce some diabetic complications such as retinopathy, nephropathy and clinical neuropathy.
Our new self-paced quality improvement (QI) activity can support general practices to use Primary Sense to identify patients with diabetes who would benefit from improved glycaemic control.
This new QI activity can provide your practice with ideas to support:
the individual review of patients
patient recall
strategies to assist in optimising education and care opportunities.

Metro South Health is currently reviewing how diabetes care is delivered across our region to ensure general practices and patients receive effective care and support.
To help inform this work, Metro South Health invites general practitioners (GPs) and practice nurses to complete a 10-minute, confidential survey.
As a thank you, survey participants will be eligible for a 50% discount to the 2026 LEADS Symposium in November.
