More support for your practice's diabetes care
General practices in the Brisbane South PHN region have room to improve their delivery of quality diabetes care to patients. The latest release of AIHW PIP QI data shows that Brisbane South PHN has a lower proportion of patients with diabetes with a HbA1C recorded in their general practice record in the previous 12 months than the national average. This indicates a significant opportunity for improvement in the management and monitoring of diabetes within the region.
Effective diabetes management is crucial for preventing complications and ensuring better health outcomes for patients. Regular monitoring of HbA1C levels is a key component of diabetes care, as it provides valuable insights into long-term blood glucose control. By improving the frequency of HbA1C recordings, general practices can enhance their ability to manage diabetes more effectively and provide better care for their patients.

Connect with the Logan Endocrine and Diabetes Service (LEADS). LEADS is hosting an open day for youth and young people living with diabetes on Saturday18 January 2025. LEADS also coordinates the Diabetes Ramadan Clinic – do you have any patients which may be suitable for referrals to this service in the lead up to Ramadan in March 2025?

GP Smart Referrals can now be used by GPs to request advice from the Metro South Health Diabetes and Endocrinology specialist service. This can support your management of patients while they remain in your care. If your practice cannot use GP Smart Referrals, a secure online enquiry can be submitted via eConsultant Request for Advice.
