National Palliative Care Week 2025 — Enhancing community-based end-of-life care

In 2025, with the support of the Australian Government Department of Health and Aged Care, National Palliative Care Week runs from Sunday 11 to 17 May.

This National Palliative Care Week, Brisbane South Primary Health Network (PHN) reaffirms its dedication to enhancing end-of-life care across the region. Aligning with this year’s theme, “Matters of Life and Death,” the PHN continues to collaborate with partners to ensure individuals receive compassionate, person-centred care that honours their choices and values.

As National Palliative Care Week 2025 unfolds, Brisbane South PHN encourages health professionals, carers, and community members to engage in conversations about end-of-life care. By fostering open dialogue and providing access to resources and education, together we can ensure that every person in our region can experience quality palliative care.

Access national resources to support quality end-of-life care at home

National Palliative Care Project caring@home have created the Community Palliative Care Resources Box—a suite of resources to aid health professionals in educating carers who support individuals wishing to die at home. The box includes practical guides, symptom management tips, and instructional videos, all designed to empower carers in providing quality end-of-life care.

The National Core Community Palliative Care Medicines List, helps ensure timely access to necessary medications, supporting both patients and carers. The Medicines List can help carers identify essential medications for managing symptoms in home-based palliative care patients.

First Nations resources to help support palliative care at home

Resources tailored to support First Nations peoples receiving palliative care in the community have been developed by caring@home and are available online. The resources are designed to respect cultural practices and provide appropriate support to First Nations people and their loved ones.

Self-care for health professionals working in palliative care

Caring for people as they near the end of life is uniquely challenging and rewarding.  Palliative Care Australia (PCA) have developed practical, ready-to-use self-care resources for busy health care professionals.
