New national resources supporting quality end-of-life care at home

National Palliative Care Project caring@home has launched its new Community Palliative Care Resources Box.

The Community Palliative Care Resources Box contains a suite of resources that can be used by health professionals to teach carers who are supporting a dying person at home.

The new Resources Box is an update of the Standard caring@home Package for Carers (first released in 2018) and is based on feedback from the health care sector. The Box contains new resources for carers and refreshes existing resources to enhance their visual appeal and readability, including:

  • tip sheets for managing physical symptoms and recognising dying
  • factsheets for understanding infusion devices
  • step-by-step guides and short videos on providing practical care
  • managing subcutaneous medicines – Information brochure, factsheets, medicines diary, wall chart, step-by-step guides, syringe labels, training checklist, videos, practice demonstration kits and sharps containers.

The Resources Box is designed to be stored centrally and contains resources that can be used with several patients and families.

  • families and carers to help manage end-of-life symptoms at home
  • patients to be cared for in the place of their choice.

caring@home resources are free of charge and are applicable Australia-wide for clinical services, health professionals, and families and carers.
