World Diabetes Day 2025—services and resources to support primary care providers

Friday 14 November is World Diabetes Day. This year’s theme is 'diabetes and well-being', putting the focus on the importance of mental and physical health for people living with diabetes.

Around 1.3 million people are hospitalised with diabetes-related conditions every year. As the first point of contact for most patients, primary care providers play a critical role in the early detection, management, and education of patients with diabetes, ensuring many never have to see a hospital.

Diabetes support from the Quality Improvement team

Our Quality Improvement (QI) team have an easy, step-by-step activity to help your practice improve the recording rates of blood pressure for your patients with diabetes.

Diabetes is a topic available for your practice to select to improve in 2026.

On Tuesday 4 November 2025, your practice received an email from Brisbane South PHN with an individual link to select your 3 preferences for quality improvement in 2026. If you haven’t completed this, please remember to complete it by 30 November 2025.

Specialist diabetes advice for patients and their general practitioners (GPs)

GP Smart Referrals can now be used by GPs to request advice from the Metro South Health Diabetes and Endocrinology specialist service. This can support your management of patients while they remain in your care.

Metro South Health’s GP Liaison Officer team are available for assistance with Smart Referrals installation and training, and enquiries about Request for Advice. Contact the team via [email protected].

Evidence-based recommendations for use in general practice—Management of type 2 diabetes: A handbook for general practice

Developed by the Royal Australian College of General Practitioners (RACGP) and Diabetes Australia, the Management of type 2 diabetes: A handbook for general practice features up-to-date evidence and advice to support high-quality care of patients with type 2 diabetes.

The handbook is available for GPs and primary healthcare providers, including practice nurses, diabetes educators and allied health professionals.
