These resources are provided to assist practices in the development of quality referrals and to allow for appropriate triage and categorisation of patients.

PLEASE DO NOT OPEN the templates until they have been imported into your clinical software as this will corrupt the coding. Word and PDF files cannot be imported into medical software.



The templates provided by us are for your personal use only and must not be redistributed without permission. They have been tested with Version of Best Practice and Version MD3.16 of Medical Director.

Metro South Health

The Metro South Health Central Referral Hub provides a single point of entry for all new referrals to:

  • specialist and allied health outpatients – Princess Alexandra, Queen Elizabeth II, Logan and Redland hospitals
  • community services previously processed by the Community Referral Service
  • oral health services.

How to refer to Metro South Health:

  • please update your fax machine for Metro South Health referrals to 1300 364 248
  • to talk to a clinical nurse or health practitioner phone the hub on 1300 364 155
  • Secure messaging via medical objects can be utilised and will be redirected to the hub.


Metro South Addiction and Mental Health Services provides community-based, inpatient, acute care and residential services which may be undertaken in the client’s home, in GP clinics or in emergency departments. Services are provided for all age groups across a range of programs from child and youth, to adult, to older persons, and specialist programs.
