Care finder program for older people at risk of falling through services
Care finders offer free, in-person support to vulnerable older people who need intensive support to access aged care and other local services.
The Program aims to support people who don’t have family members, friends, a carer or a representative to help them access aged care services and supports. However, the care finder service is not available for everyone. Learn more about eligibility here.
Care finders can help eligible older people to:
- understand what aged care services are available to them
- set up and attend assessments
- help choose appropriate services.
How to connect your patients with a care finder provider

When you contact a care finder provider, you will need to answer some questions about why your patient needs help from a care finder. The patient must give consent for you to provide this information, so it is best if they are with you when you call.
