New self-paced QI activity to support preventative health in patients aged 45 to 49

Brisbane South PHN has launched a new self-paced quality improvement (QI) activity focused on Health Assessments for patients aged 45 to 49, designed to help general practices strengthen the delivery of preventive care.

As part of the activity, practices will identify and invite 10 eligible patients to participate in a health assessment over a 3-month period. The flexible, self-paced format allows practices to complete the activity at a time that suits their workflow while improving the quality and consistency of preventive health checks. Ultimately, this service aims to enhance patient health outcomes through a proactive, targeted approach.

Contact the General Practice Quality Improvement team if you would like more information on this activity via [email protected]

Contact the QI team [email protected] or 3864 7540 for more information.
