Health Needs Assessment Survey

Brisbane South PHN and Metro South Health work together to provide health programs and services to the people in our shared region. In 2024, we're partnering to jointly assess the health needs of the people who live and work in our region. This Health Needs Assessment Survey forms part of an extensive regional Needs Assessment undertaken every 3 years.

Commonwealth and state governments mandate Health Needs Assessments to ensure resources and funding to improve population health and wellbeing outcomes are being used most effectively. Answers to this survey help Brisbane South PHN and Metro South Health gain a deeper understanding of current health needs and the service gaps experienced across our vast and diverse region.

Yes, your privacy is important to us. Your responses will be de-identified and will not be reported at the individual level. Your responses will be combined with responses from other individuals to identify common themes that arise from our survey results.

Contact details provided to enter the voucher draw will be used solely for that purpose and not shared with anyone outside of Brisbane South PHN, in accordance with our data protection and privacy practices.

Only members within Brisbane South PHN will have access to the raw survey results. Themes and de-identified analysis will then be shared with Metro South Health. Data and responses that you provide to us will be stored electronically on the secure Brisbane South PHN system. Survey Competition terms and conditions

1. Brisbane South PHN (Primary Health Network), at 2404 Logan Rd, Eight Mile Plains, Queensland 4113, is conducting a prize draw for respondents of the Health Needs Assessment Survey.
2. Entry is open to all respondents of the Health Needs Assessment Survey.
3. Submitting a survey fulfils the conditions of entry into the competition.
4. Only one entry per person is permitted. Multiple submissions of the same survey or from the same person will not result in additional entries into the prize draw.
5. To enter the competition participants must complete an online survey at https://surveyresearch.com.au/n/qgnSaQp?ver=1496675114 and include their name and preferred contact details at the end of the survey, where indicated, to enter the draw.
6. There will be three draws conducted at the end of the survey period. Details are as follows:
   a. For all draws, entries close Tuesday 30 April 2024 at 9am. Draws will take place on Wednesday 1 May 2024.
   b. All draws will take place at Brisbane South PHN offices. Winners will be selected randomly by using a random number generator.
   c. All winners will be notified via the contact details they provided in their survey response.
7. There will be three winners drawn, each winner will receive a $250.00 store voucher.
8. Total maximum prize pool is valued at $750.00.
9. The winner will be notified about how the prize will be delivered (or may be collected) upon confirmation of the award of the prize.
10. Personal Information, for the purposes of the Privacy Act 1988 (Cth), means information or an opinion about an identified individual, or an individual who is reasonably identifiable, whether the information or opinion is true or not and whether the information or opinion is recorded in a material form or not.
11. By entering this Promotion, the entrant’s consent to the use of their personal information for the Purpose.
