Media Consent and Release Declaration

Brisbane South PHN seeks your consent to use photos, audio or video recordings taken by us of you, or quotes or written testimonials you supply us with (Materials) for use in our public communications.

Important: Where you sign this consent form as the parent or guardian of someone aged less than 18 years (Minor) you consent, on behalf of the Minor, to the collection and use of the materials as set out below.

  • I hereby consent to Brisbane South PHN (including its employees and contractors)  taking, recording, publishing, reproducing or utilising the Materials ) in any medium and for any purposes Brisbane South PHN considers appropriate (provided such use is connected to Brisbane South PHN’s operations).  This may be in print, visual, online and social format, including Brisbane South PHN’s website or online services, third-party websites or applications (such as social media or video-sharing websites). 

  • I consent to Brisbane South PHN using my name or other personal information regarding me (or, where applicable, the Minor) for the purposes set out in this consent form.   

  • I acknowledge that the consents given in this form and the rights of Brisbane South PHN in relation to the Materials cannot be revoked. 

  • I acknowledge that I (or, where applicable, the Minor) will not be entitled to any royalty, fee or other payment or consideration in relation to Brisbane South PHN use of the Materials in accordance with this consent form. 

  • I acknowledge that I will not own any copyright or other similar rights in respect of the Materials. 

  • I represent that I am legally able to sign and have read and understood the terms of this consent.  Where I have signed this consent form on behalf of a Minor, I represent and warrant that I am the parent or legal guardian of the Minor. 

Please be aware that: 

  • Any personal information published on the Internet is fully accessible to anyone with internet access. 

  • While the Brisbane South PHN website is protected by copyright, once information has been published on the internet, Brisbane South PHN has no control over its subsequent use or disclosure.

  • I accept responsibility for notifying Brisbane South PHN of any changes or required updates to personal details or information. 

  • I also consent to the use of related photographs, video, other media, or written statements used for business purposes of/by me to be used by third parties involved in this event/activity for related purposes. This could also include, but may not be limited to, events at third-party venues, internal and or external properties, etc. 

Brisbane South PHN is committed to ensuring that your personal and health information is handled in accordance with Australian privacy laws. All Brisbane South PHN employees are bound by the legal requirements of the Australian Privacy Principles from the Privacy Amendment (Enhancing Privacy Protection) Act 2012.
