Media Consent and Release Declaration

Brisbane South PHN seeks your consent to use our photos, audio or video recordings taken of you, or quotes or written testimonials you supply us with for use in our public communications. Please carefully read through our terms for media consent below. If you have any questions for us about these terms, contact communication@bsphn.org.au.

  • I hereby consent to Brisbane South PHN taking, recording and utilising photographs, video, other media or written statements used for business purposes of/by me (and/or work related property) to use in any Brisbane South PHN media that may include print, visual, online and social, now or hereafter.

  • This consent may include the use of my name or other personal information that may be revealed therein or by descriptive text or commentary. I do hereby release to Brisbane South PHN the rights to exhibit for business or promotional purposes this work in related media including but not limited to print and electronic forms.

  • I agree that any use described herein or hereafter may be made without compensation and or additional consideration.

  • For products and materials containing sensitive topics Brisbane South PHN will seek additional approvals for media that may include print, visual, online or social.

  • I declare that I have been informed about the nature and use of the information and statements I have supplied to Brisbane South PHN.

  • I represent that I am legally able to sign and have read and understood the terms of this consent.

Please be aware that:

  • any personal information published on the Internet is fully accessible to anyone with internet access

  • the Brisbane South PHN website is protected by copyright, however once information has been published on the internet Brisbane South PHN has no control over subsequent use or disclosure

  • I accept responsibility for notifying Brisbane South PHN of any changes or required updates to personal details or information

  • I also consent to the use of related photographs, video, other media or written statements used for business purposes of/by me to be used by Third Parties involved in this event/activity for related purposes. This could also include but may not be limited to events at Third Party venues, internal and or external properties etc.
