Brisbane South Primary Health Network (PHN) and Metro South Health are partnering to assess the health needs of the people who live and work in our region. The joint Health Needs Assessment Survey is part of an extensive regional Needs Assessment undertaken every 3 years.
Commonwealth and state governments mandate Health Needs Assessments to ensure resources provided to improve population health and wellbeing outcomes are being used most effectively. Answers to this survey will help Brisbane South PHN and Metro South Health gain a deeper understanding of current health needs and service gaps experienced across our vast and diverse region.
All health professionals working in the Brisbane South PHN region are welcome to take the survey and encourage their patients to do the same.
What is the Health Needs Assessment?
It's a tool we use to understand the health and service needs of the community.
This survey helps identify health priorities based on how the people in our area use the health system.
This means we can distribute resources more equitably and plan programs that cater to the specific needs of our residents.
Why have a Health Needs Assessment?
As a PHN, we work towards 2 key objectives:
• Improve the efficiency and effectiveness of medical services, particularly for those most at risk of poor health outcomes.
• Better coordination of care so people receive the right care in the right place at the right time.
